The California Consumer Privacy Act, including as amended by the California Privacy Rights Act (the “CCPA”), gives California residents certain rights with respect to their Personal Information (“PI”) that we maintain, which they may exercise by submitting a rights request to us using the form below. If you are not a California resident, you are not entitled to exercise such privacy rights.

As explained in greater detail in Section 2 of our California Privacy Notice, California residents have the rights to access PI we maintain about them, delete PI that we have collected from them, and correct PI that we maintain about them. Please complete this form to submit a rights request. We will respond to your request as required by the CCPA.

If you are an authorized agent making this request for someone else, please complete Sections 1, 2, and 3 of the below form on behalf of the person for whom you are making the request.
